Scandal pg: 178 in LotW
"Acting foolishly or making heinous mistakes draws the attention of the Ragabash, who may bring up the matter during Renown awards. The Ragabash may then attempt to negate the award by speaking scandalously about the recipient, pointing out his less-than-stellar acts (especially if they were performed to acquire the very Renown the target is being awarded). The scandal must have an element of truth in it, and must be agreed to by the Ragabash elder of the sept (so the accusing Ragabash must have some proof). It is up to the Galliards to confirm the scandal and agree that the Renown should be lost. Only one Renown award may be affected per scandal"
As of the above, it means that a Ragabash can try to make a renown hit on someone at the time of the Rite of Accomplishment. Scandal means only a renown hit, and will not cause any other punishment then that.
Now. IMPORTANT
When a character actually do an act that requires a punishment it is up to anyone (not only a Ragabash) to report it to the local Philodox. No one can stop you, even a "no" from an above rank. It is completely up to the person if he wants to hunt it or not, and there is no special procedure to do it. Although, if the act is questionable, or there is no obvious proofs of the guilt of the accused, A ragabash must be asked to find them and report to the Philodox that is in charge of the case.
A trial then will be done by the Philodox (in serious cases by the Elder Philodox of the Sept) and a suitable punishment will result.
Note: Although a character may lose ranks, he can not go below Cliath. The renown may go on a negative number, but ONLY the Totem of the Tribe himself can actually turn the Garou into a cub again.
